Privacy Policy

This policy explains what this website collects and how that information is handled.

This policy is separate from our Notice of Privacy Practices, which describes how Kathleen Fugere, ARNP handles protected health information for patients under HIPAA. If you are a patient asking about your medical record, please see the Notice of Privacy Practices.

What this website collects

Contact form

The form on the Schedule page asks for your name, email address, phone number, preferred contact time, and a message. Submissions are stored by Webflow, the service that hosts this website, and a notification is emailed to Kathleen. This information is used only to respond to you.

The contact form is not a secure channel. Please do not include medical, clinical, or personal health details in it. Anything private can be shared through the patient portal instead.

Appointment request calendar

The Schedule page includes an appointment request calendar provided by CharmHealth, the practice's electronic health record service. The calendar loads directly from CharmHealth, so what you enter in it, such as your name and contact details, goes to CharmHealth rather than to this website. Once submitted, a request becomes part of the practice's records and is covered by our Notice of Privacy Practices.

Technical information

As with any website, our host receives routine technical information with each visit, such as your IP address, browser type, and the pages you view, in order to deliver the site to you.

What this website does not do

This website uses no analytics, advertising, or tracking cookies, and carries no tracking pixels. The only cookies are ones needed for the site and the appointment calendar to work: a security cookie set by the network that delivers this website, and session cookies set by CharmHealth when the calendar loads. We do not sell your information, and we do not share it with advertisers or data brokers.

Other services we use

Webflow hosts this website. The appointment request calendar and the Patient Portal are both provided by CharmHealth, a separate service with its own privacy practices. Anything you enter in the calendar or the patient portal is handled by CharmHealth under its own terms, not by this website. Text messages from the practice are sent through Spruce Health.

Text messages

If you agree to receive text messages from the practice, your mobile number is used only to send you those messages. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent will not be shared with any third parties. How long we keep information

Contact form submissions are kept for 12 months and then deleted. If you become a patient, information relating to your care is kept in your medical record instead, and is governed by our Notice of Privacy Practices rather than this policy.

Your choices

You are never required to use the contact form. You can call or email Kathleen directly instead. You may also ask what information from a form submission we hold, and ask us to delete it, by contacting us at the phone number or email address above.

Children and young people

This website is not intended for use by anyone under 18. Kathleen sees children and adolescents as patients, but if you are under 18, please ask a parent or guardian to contact the practice on your behalf rather than using this form yourself.

Changes to this policy

If this policy changes, the updated version will be posted on this page with a new date at the top.
